What Prata Handles, and What We Do Not

Clear scope protects your patient and your name on the referral. Here is exactly what we take on, and where we stop.

We provide skilled nursing in the home led by a Registered Nurse: medication management and reconciliation, wound care, basic IV therapy, vitals and clinical monitoring, post-operative recovery nursing, chronic condition management for conditions such as CHF, COPD, and diabetes, and full health navigation so the patient and family are not coordinating specialists, pharmacies, and appointments on their own.

  • Skilled nursing at home, RN-led, with a Registered Dietitian, PharmD, and Nurse Educator on the team for complex cases.
  • Medication management, basic IV therapy, wound care, and clinical monitoring between your visits.
  • Health navigation: coordinating specialists, appointments, pharmacy, and records so nothing is dropped.
  • We do not provide advanced or complex IV therapy, tracheostomy care, neonatal care, or event nursing. If a patient needs those, we will tell you up front so you can place them appropriately.

The Clinical Handoff

A referral is only as good as the handoff behind it. Ours is built so nothing is lost between your discharge and the patient's first day with their nurse.

When you refer, an RN reviews the patient's needs, confirms the plan of care, and establishes a single point of clinical contact for your office. You tell us what you want reported and how often. From there, the patient has one nurse leading their care, and you have one nurse to call. No phone tree, no rotating staff who have never seen the chart.
